Verapamil:

Calcium Channel Blocker (Non-DHP type)

   

 

IVP DOSING

   
5-10 mg

Give slow IV over at least 2 minutes...  Repeat again in 15 to 30 minutes if needed.

   

30 mg

You gotta stop now and try something else...
 

INFUSION DOSING

   
If it works nicely, keep it running. 5-10 mg/hr
 

Verapamil inhibits calcium mediated depolarization in the AV node, decreasing conduction,  and inhibits the influx of calcium across cardiac and smooth muscle cell membranes, decreasing O2 demand by decreasing myocardal contractility.  It has the desired non-DHP effect of equally dilating both arteries and veins in both coronary and peripheral vessels.  It is used for PSVT, rapid A fib/flutter, HTN, vasospastic angina (Prinzmetal's variant), and stable angina.

 

Side Effects: AV block, ¯ HR, ¯ BP, VF, asystole, CHF, pulmonary edema, dizziness, HA, nausea, rash, elevated liver enzymes.

 

Contraindicated: LV dysfunction, VT, 2° or 3° AV block, SSS, WPW, short PR, ¯BP, shock, IV b-blocker use.
