Propranolol:

Non-Selective b-Blocker

   

 

IVP DOSING

   
1-3 mg

Give no faster than 1mg per minute!  Continue to give every 5 minutes as needed until a maximum dose of 0.1 mg/kg has been reached.

 

INFUSION DOSING

   

This IS the dose range for a continuous infusion.

1-3 mg/hr
 

Propranolol is a non-selective b-blocker, used in a wide variety of situations including VT/VF , A fib/flutter, PSVT, HTN, angina, anxiety states, essential tremors, and prevention of migrains.  Also used for acute anterior MI when there is ­ BP & ­ HR, big AMIs < 6hrs old, and chest pain or tachycardia that is due to excess sympathetic tone and stimulation.  It reduces myocardial O2 demand, ¯ BP, ¯ force of contraction, and ¯ renin which prevents cerebral arterial vasodilation (¯ migraines).  It undergoes extensive first-pass hepatic metabolism, and is highly lipid soluble, allowing it to easily pass through the BBB.  Its adverse CNS effects make it less tolerable when ongoing therapy is required.  It has a short half-life, and must therefore be given BID when in oral form. 

 

Side Effects: are ¯ BP, ¯ HR,  CHF, angina, acute pulmonary edema, bronchospasmdizziness, HA, N/V, fatigue, vivid dreams and/or insomnia, hallucinations, depression, and dizziness.  CCBs may  ­ SEs.  

 

Contraindicated: in pts with CHF, ¯ BP, ¯ HR, 2° or 3° AV block, or cardiogenic shock.
